AHDSS was created with the aim of supporting district health systems development led by the post-apartheid Ministry of Health in South Africa. The HDSS includes all residents of all ages of the Agincourt sub-district of the Bushbuckridge district in Mpumalanga Province in northeast South Africa near the border of Mozambique. Temporary in or out migrants are eligible to join the cohort if they reside in the study area for more than six months of the year and retain close ties to the community. At baseline in 1992, the study population included over 57,000 people within 8,900 households in 20 research villages. Over time, the study has expanded to include 30 research villages with a population of 107,500 people, living in 19,500 households as of 2013. Participants are followed up with up to three times per year.
