The Cracow Stroke Database is a hospital-based, prospective cohort established to investigate the epidemiology, risk factors, and outcomes of stroke among patients treated at the Department of Neurology, Jagiellonian University Medical College, in Krakow, Poland, and includes data on over 1,300 consecutively admitted stroke patients. Participants were recruited consecutively from hospital admissions beginning in 1994, with eligibility restricted to adults aged 40 years or older. The initial 1994 to 2000 cohort included 816 first-ever stroke patients and a subsequent cohort of 569 patients was enrolled between 2008 and 2012 to assess temporal trends in stroke severity, survival, and vascular risk factors. In addition, a subset of 250 patients admitted from 2000 to 2001 was studied for pre-stroke dementia risk, forming a smaller, cognitively focused sub cohort. Participants are followed up through hospital discharge, 30 days, and 1-year post-stroke, with additional cognitive follow-up at 3 months in the dementia-related sub study.
