The KCRHS is a prospective cohort study of a large stratified random sample of residents in one rural agricultural county (Keokuk) in Iowa, United States. Each member of selected households within Keokuk County were invited to participate in the study. Farm and rural non-farm households were oversampled since the study focused on rural and agricultural health. KCRHS enrolled and surveyed over a thousand households during Round 1 (1994 to 1998) and Round 2 (1999 to 2004), and concluded with 662 households in Round 3 (2006 to 2011).
