The SSNAP is a nationwide stroke surveillance initiative that monitors the quality of stroke care across England, Wales, Scotland and Northern Ireland, with 95,222 stroke admissions recorded as of the 2023/24 reporting year. The cohort includes all patients admitted to hospital with a diagnosis of stroke, beginning in 2012, and is continuously updated through routine clinical data submitted by participating hospitals. Admissions are continuously monitored via linkage to clinical data, and participants are also followed up once at 6 months post-admission, with outcome measures collected to assess recovery and service quality.
