The TorSaDE aimed to identify the most effective and efficient care trajectories for people with chronic conditions. The cohort included over 80,000 residents in Québec (Canada) aged 12 years and over who participated in at least one of the four cycles of the Canadian Community Health Survey (CCHS) conducted between 2007 and 2014. The TorSaDE data encompasses all available information from the CCHS questionnaire and linked medico-administrative data spanning a 21-year period (1996–2016).
