The Wosera HDSS was initially set up in 1990 as part of a large Malaria Vaccine Epidemiology and Evaluation Project in Papua New Guinea. It aimed to study malaria epidemiology and immunology in nine villages. In 1995, it expanded to cover 30 villages in the Wosera/Gawi district, in the East Sepik Province of Papua New Guinea. As of 2011, the Wosera HDSS included over 16,000 people. The data collected included yearly population censuses, fortnightly updates, verbal autopsies, and morbidity surveillance. Additional data was collected as required by sub-studies.
